Direct primary care may be exactly what you are looking for! With a small start up fee and a monthly low-cost “membership”, you are able to receive the health care you deserve. Walk-in visits, video or phone call visits, same day appointments, and an ample amount of time spent with your provider is what we are talking about here.

Payment
For the membership, a credit card for automatic draft on the 1st of every month is required. For all other services, we accept cash and major debit/credit cards.
*Pricing is always discussed with patient prior to visit, but prices are subject to change based on the nature of the visit.

- WHAT IS DIRECT PRIMARY CARE?

In the simplest of forms – direct primary care (DPC) is a way of healthcare that was always supposed to be, -simple, convenient, personal, + affordable. On this DPC website it is explained very well – take a look.
- IS THERE A CONTRACT OR COMMITMENT? WHAT IS THE CANCELLATION POLICY?

We have a minimum of a 3 month commitment. You must notify us 30 days in advance to cancel your membership. Monthly dues paid will not be refunded if the membership is canceled during that month. *There is a one-time $99 start-up fee for members.
- WHAT ARE THE TYPICAL SERVICES THAT ARE PROVIDED WITH MY MEMBERSHIP FEE?

Included is a membership: primary care services to include unlimited in person, telehealth, or phone appointments, no co-pays, annual physical, annual well woman exam, strep test, flu test, urine drug screen, simple laceration repair, as well as suture removal. In addition, members can receive many services at Restoration Wellness at a discount.
- WHAT IF I HAVE INSURANCE?

Perfect! We advise patients to carry some form of insurance that will cover services that are not covered in your membership – such as an ER visit, hospital stay or major surgery to name a few. The insurance you have will continue to pay for services provided outside of Restoration like imaging, labs (if you wish), medications, specialists visits, etc. We do offer discounts for most medication that you can get through our clinic, as well as labs.
- DO YOU ACCEPT PATIENTS WITH ALL MEDICAL CONDITIONS INTO YOUR MEMBERSHIP PROGRAM?

Everyone is unique + different. Therefore, this is determined on an individual basis. Carrying a different kind of insurance than mentioned above may be what is best for you depending on your conditions. If you have questions or concerns, please contact us!
- WILL I STILL BE REQUIRED TO PAY MONTHLY EVEN IF I DON’T USE ANY SERVICES THAT MONTH?

Yes, you will be still be required to pay the monthly membership fee. The monthly membership allows you to be able to access the services provided in your membership with same or next day appointments in office or video/phone call when they are needed.
- WHAT IF I CANCEL AND WANT TO RE-ENROLL?

We would love to have you back. We cannot promise any open spots at the time you wish to re-enroll. We only take a certain amount of patients to offer the best, convenient care to our members as possible. If there are no spots available, we can place you on a waiting list!
- WHAT IS ONPATIENT?

The onpatient portal is an easy way to manage your personal health records. Benefits of the onpatient portal include:

- WHAT IS A FAMILY NURSE PRACTITIONER OR OTHERWISE KNOWN AS A FNP?

A family nurse practitioner (FNP) is a licensed, independent medical practitioner who works in a variety of healthcare settings. NPs provide a wide range of health care services including the diagnosis and management of acute, chronic, and complex health problems, health promotion, disease prevention, health education, and counseling to individuals, families, groups and communities, among more. NPs hold a Masters Degree in Nursing. In Tennessee, NPs work under the supervision of a physician. Research has shown that patients who see a NP stated that they received high-quality care and were very satisfied. Read more about that here.

- WILL I EVER SEE THE PHYSICIAN?

No. The supervising physician at Restoration does not see patients at this practice, but she helps us ensure the best care. She is always available for consultation or questions when needed.
- WHAT KIND OF SERVICES ARE OFFERED AT RESTORATION WELLNESS?

We offer preventative and wellness care as well as care for minor acute illnesses such as flu illnesses, sinus infections, allergy symptoms, urinary tract infections, pink eye, skin irritations, minor laceration repair, suture removal, incision and drainage, ear wax removal, and more! We also draw labs in our office and offer discounted lab prices, as well as some prescriptions.
We also offer IV therapy, vitamin injections, weight loss programs, bio-identical hormone replacement, laser hair removal, laser skin rejuvenation, body sculpting, incontinence treatment, and botox and filler aesthetic services.
- DOES RESTORATION ACCEPT INSURANCE?

To offer our patients the best care possible, we do not accept insurance (this includes Medicaid and Medicare). If you have health insurance, insurance can be used for services provided outside of Restoration (specialty services, referrals to other providers, imaging, etc).
- DO I HAVE TO BE A MEMBER TO BE SEEN?

No! Urgent care or acute care walk-ins are welcomed. A non-member is also welcome to take advantage of our aesthetic, IV therapy, injections, hormone replacement, or weight loss services. However, to consider Restoration as your primary care provider, membership is required. If you are interested in a membership, stop by our clinic and we would love to answer any questions that you may have.
- WHAT IF I AM OUT OF TOWN AND NEED A DOCTOR?

Contact us! We can connect many of ways (Onpatient Patient Portal, Phone, FaceTime, email, text) to discuss your situation. Many minor illnesses can be treated over the phone. If medication is needed, we’ll help locate the closest pharmacy to you to send your medicine.
- WHAT IF I DON’T HAVE INSURANCE?

That is ok. Insurance is not required nor accepted. We do encourage patients to carry some form of insurance that will cover services that are not included in your membership. An insurance plan that is a high deductible, low monthly premium plan or catastrophic health care plan that would cover you for any unforeseen, expensive circumstances may be a good choice for you. For patients who do not carry a private insurance plan, we can help you with wholesale pricing for labs and most medications.
- DO I HAVE TO PAY EVERY TIME I COME IN FOR A VISIT?

There are no office co-pays. Everything is included in your monthly membership fee. However – for services provided that are not included in the membership, there are separate fees. Non-member visits are set at specific rates along with needed services provided during the visit, as stated above. Think of the membership like any other type of membership. Whether you use none or most of the services offered that month, you still will be required to pay the monthly fee. For non-members, price is for every visit.
